Yes. Simply have your dentist or dental office send an email to SavingsClub@MoonlightGraham.com requesting to join the Moonlight Graham network. We will follow-up to vet the dentist and dental office to confirm credentials and good standing, and then if appropriate we will provide the information required to join the network.

The Moonlight Graham dental savings club is NOT insurance. This is not a qualified health plan under the Affordable Care Act (ACA). This plan does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00. This is not a Medicare plan. The plan provides discounts at participating dental care providers for services. The plan does not make payments directly to providers. The member will receive a discount from participating dental care providers, and the member is required to pay for all the dental care services received from the provider. The range of discounts will vary depending on the type of provider and services. The licensed Discount Plan Organization is Moonlight Graham of Wisconsin, LLC. 10201 N.
